$40 for an Individual

$60* for a Couple - *Must be seen together as a couple at the first appointment with the same practitioner.

$75** for a Family (includes 1 child) then $15 for each child up to 5 children that are residing in the residents, if more then 5 we can discuss at enrollment.

**Family consist of husband, wife, natural or adopted children up to age 20 (or 26 if full-time student). The term spouse shall mean the person recognized as the husband or wife under the laws of the state where they live in.

Children shall include natural, adopted, or children who are subjects of suits for adoption to which a covered family member is the party of. Step children, who reside in the family household, may also be included, as long as the natural parent remains married and also resided in the family household. Legal Guardian of a child or children if they resided in the family household and present legal documentation.
